When we create the QR Code for you it is checked that it contains all the information that you've requested at the time of ordering and it's working.
If when you receive your tags, the QR CODE cannot be scanned or show's only one line of the information that you've entered when ordering or just a phone number, this is usually down to your particular phone model inbuilt camera / software not being able to read QR Codes through the native camera app or phone software. This is not an error with the QR Code or Tag nor with the phone.
If your phone isn't able to read all QR Codes natively don't worry, there are many apps that you can download to your phone that will allow your phone to scan QR Codes for free from the "apple app store" if you have an iPhone or from the "Play Store" if you use an android phone that will allow you to scan QR Codes. When you use these you will see all the information that you requested on your tag is included in your QR Code.
